What this website collects

The consulting and PLRM beta forms collect the contact, organization, role, program context, needs, timing, consent, and no PHI acknowledgment you choose to provide. When present, the site also stores campaign parameters and the referring page so Perception of Life can understand where an inquiry came from.

How the information is used

Inquiry information is used to review fit, respond to the request, plan a possible consulting conversation or beta follow up, and evaluate whether outreach is reaching the right people. If you separately choose the optional consent for future offers, Perception of Life may also contact you about relevant services, tools, or beta opportunities. It isn’t sold as a lead list.

Who can review it

Records are stored in the website’s controlled database and are available through a private, allowlisted owner workspace. Access is limited to the authorized Perception of Life owner account and the infrastructure needed to operate the site. Email notifications are currently disabled, so the owner reviews saved inquiries manually.

Do not submit protected health information

Do not enter client names, dates of birth, record numbers, clinical notes, diagnoses tied to an identifiable person, or other protected or identifying clinical information. These forms are for business and beta interest inquiries, not treatment, crisis support, or clinical record transmission.

Retention and removal

Inquiry records are retained indefinitely so Perception of Life can preserve business history, evaluate long term interest, and identify relevant future opportunities when optional consent was provided. You may request correction, removal, or an end to contact about future offers at any time by emailing junior@perception-of-life.com.

First-party website analytics and campaign tracking

Perception of Life measures public website page views, landing pages, video impressions, video starts and viewing milestones, primary link clicks, form starts, and successful inquiry submissions. This first-party measurement also preserves campaign parameters, a limited referrer host and path, the related video or placement, a device-size category, and a random session identifier that ends with the browser session.

The analytics event system doesn’t collect names, email addresses, organizations, inquiry narratives, client information, protected health information, complete IP addresses, or full browser fingerprints. It doesn’t use advertising pixels, build cross-site profiles, sell behavioral data, or track activity inside the protected PLRM portal. Measurement stops when a visitor follows the public portal sign-in link.

Raw analytics events are retained for 13 months and are then removed or converted into monthly totals without session identifiers. Anonymous campaign and conversion totals may be retained to understand long-term business trends.

PLRM and clinical judgment

PLRM supports preparation and structured drafting. It doesn’t replace clinician review, clinical judgment, supervision, emergency procedures, or the specific legal and regulatory responsibilities of an organization.
